As you will not be able to guarantee this arriving in the post anymore, you should use the Word version of the form attached here (2015-16 CDTTL Individual Championships Entry Form.docx) and email this back me at arthurcclark@btinternet.com.

Late applications will not be accepted.

Payment of your entry fees can also be made electronically (details on the Entry Form) directly into the C&DTTL bank account.
