Team captains/players - please let your club secretary know of your team entry/player details so that they can submit the club's entries to me. Don't forget to advise them of any change of address, email or mobile/ home / work telephone numbers.
PLEASE COMPLETE THE DETAILS IN CLEAR LEGIBLE HANDWRITING, TO AVOID ERRORS IN THE HANDBOOK - THANK YOU.
The completed form/s along with a minimum deposit of £15.00 per team, need to be returned to me by Monday 31st July 2017. Please also see attached letter, for further information and payment details.
Individual player entry forms will be sent out with the handbooks in September.
If you should have any further queries, please let me know. It would be also be helpful if you could let me know, if you are not intending to enter a team in 2017/18, that has played last season, thank you.
